Licensing authority forms for Scotland
It is a statutory requirement that applicants use the correct forms to give proper notice of applications, variations and so on, to all responsible authorities, including the Gambling Commission.
Licensing authorities must also use the correct statutory forms and notices for the grant and/or rejection of applications (new, variations, transfers, and so on).
You also have statutory duties to notify the Gambling Commission as well as the applicant and other responsible authorities of the grant and/or rejection of applications (new, variations, transfers etc) as well as the revocation, surrender or lapse of a premises licence using the correct statutory forms and notices.
We have brought all the statutory forms into one place to help you to comply with those statutory processes.
The statutory forms and notices which are for use in Scotland are subject to different Regulations and these can be found on this page.
